PARENT CONSENT FORM FOR PHOTOGRPAHY AND VIDEOGRAPHY 

 

I give permission for Studio B Performing Arts Centre to use video footage and / or photographs of my child. This usage may include (but is not exclusive to) distributing, publishing, and displaying images and/or videos publicly for the use in:

• printed materials (eg – brochures/Flyers)

• online advertising and promotion

• videos and digital images such for use on social media.

By signing this form, I acknowledge that I am giving unrestricted permission for my child’s image to be used in print, video, and digital media. First names may be used but we shall not display surname.

Studio B can assure you that all materials will be used exclusively by us and will not be passed onto any third parties.
